Our appointment request and new patient intake forms are provided by Weave, an outside service, and open in a new browser tab on Weave's own website (forms.wv3.io). Any information you enter into those forms is submitted directly to that service and to our practice, and it is handled under Weave's privacy practices and our own office privacy practices. It is not collected or stored by this website. Please avoid including detailed health information in a general message; you can share medical details securely with our team at your visit.
The Smile Quiz
Our homepage includes a short quiz that points you toward the service that may fit your concern. The quiz runs entirely in your browser: the answers you tap are processed on your device to show your result, and they are never sent to us, stored by this website, or shared with anyone. If you choose to ask our office to reach out at the end of the quiz, you are taken to our secure request form provided by Weave, the same provider that handles our appointment requests, and your details are handled under Weave's privacy practices and our own office privacy practices; your quiz answers are not included. The quiz is for general information only. It is not dental or medical advice, a diagnosis, or a treatment recommendation, and using it does not create a patient relationship.
Online payments
If you pay a balance online, you are taken to a secure payment portal hosted by our payment processor, CardPointe (CardConnect). Your card details are entered on that processor's secure page. This website does not process, see, or store your payment card information.
Cookies
Our website does not use analytics or advertising cookies, and it does not run third-party trackers. Fonts are served from our own site. The only other third-party content is the embedded Google Map on our Visit pages, which loads as you scroll to it, and Google may set its own cookies when it does. Our appointment forms (Weave) and online payments (CardPointe) open on their own pages under their own policies. You can control cookies through your browser at any time. How do we use the information?
